Is a vote of no confidence usually attached to legislation or is it called for separately?

if not, what is it called when attached to a bill?Is a vote of no confidence usually attached to legislation or is it called for separately?A vote of no confidence is basically a parliamentary act. It is used to embarrass the current party in power. A vote of no confidence is then followed by a constitution convention.



I know of no such scenario for American Politics. Perhaps it could be a tool used against an individual. But that is only a guess on my part.
